The answer with the conjugations would be:
Usted salió antes de las cinco.
You went out before five.
Since (ud.) is singular for formal you you would take away the -ir from the stem and add -ió.
If it was (Uds.) then it would be more that one person and would be:
Ustedes salieron antes de las cinco.
